STUDENT COMMITTMENT

  • I have read and understood the Adapted Exam Procedures, and commit to respecting conditions as outlined in these procedures.
  • I understand that in order to be eligible for adapted exam measures I must have appropriate documentation on file and that it is my responsibility to ensure that the documentation is accurate and up to date and that I contact my Learning Specialist at least once a year to confirm that my file is active.
  • I authorize Access Service to disclose my adapted measures to my professors/Faculty/Student Services.
  • I acknowledge that any violation of the University's policy on Academic Fraud will be reported to my Faculty.
I agree to the Student Committment Policy as listed above.
