Mandatory Documentation
There are different documentation requirements for different needs:
- ADD or ADHD: documentation from your doctor or psychologist.
- A Learning Disability: a psycho-educational assessment no more than 5 years old.
- A Physical Disability, or A Temporary or Chronic Medical Condition: documentation from your doctor, audiologist, occupational therapist, physiotherapist or other health specialist.
- A Psychiatric or Psychological Condition: documentation from your doctor, psychologist or psychiatrist - specifying a DSM-IV-TR diagnosis code.
- A Traumatic Brain Injury: a neuro-psychological assessment report.
The documentation you provide must answer the following questions:
- What is the current diagnosis?
- Is the condition permanent or temporary? If the condition is temporary, state the duration for the validity of the documentation.
- If applicable, what is the severity and duration of the symptoms?
- How does the disability (including medication side effects, if applicable) impede or impair academic functioning (e.g., mobility on campus, attending lectures, note taking and test taking)?
- What are recommended adaptive measures for the classroom and for evaluations?
Medical Certificate
Download the PDF version of the Medical Certificate. This form should be filled out by your doctor and returned to Access Service as soon as possible. You will be responsible for paying any doctor’s fees related to the completion of this form.
If you don’t have any documentation, or if it requires an update…
Access Service specialists will examine potential funding sources with you and then make the necessary arrangements to have the new assessment completed by the end of the semester.
In the meantime, your Individual Education Plan from high school (if you have one) and the functional evaluation done by the Access Service specialist during the intake interview will provide strong indicators to determine what temporary adaptive measures can be put in place.
Medical Certificate for Housing Services
Students requiring an adapted housing unit because they have a disability or special needs, linked to a medical condition should contact Access Service, who will validate the request with Housing Service with the necessary supporting documentation.
